Health Assessments

The Department of Health encourages preventative Health Assessments for the following key groups.

There are no out-of-pocket costs for any of these assessments.

45-49 year old Health Assessment

A health assessment at this stage of life can assist patients to make the necessary lifestyle changes to prevent or delay the onset of chronic disease.

People 75 years and older

The purpose of this health assessment is to help identify any risk factors exhibited by patients over 75 years of age that may require further health management. This health assessment is used to identify factors that influence a person's physical, psychological and social functioning and is conducted on an annual basis.

People with an Intellectual Disability

The health assessment provides a framework for doctors to comprehensively assess the physical, psychological and social function of patients with an intellectual disability and to identify any medical intervention and preventive health care required.

Aboriginal and Torres Strait Islanders

The aim is to help ensure that Aboriginal and Torres Strait Islander people receive health care matched to their needs, by encouraging early detection, diagnosis and intervention for conditions that cause morbidity and early mortality. The health assessment for Aboriginal and Torres Strait Islander people covers the full age spectrum.

Humanitarian Entrants

The purpose of this health assessment is to introduce new refugees and other humanitarian entrants to the Australian primary health care system, as soon as possible after their arrival in Australia (within twelve months of arrival).  The health assessment applies to humanitarian entrants who are resident in Australia with access to Medicare services.

Former serving members of the Australian Defence Force

A health assessment for a former serving member of the Australian Defence Force is an assessment of a patient's physical and psychological health and social function and whether health care, education and other assistance should be offered to the patient to improve their physical, psychological health or social function.
